Required for all Scouts and adults participating in any troop activity or weekend campout. This form is completed by a parent or guardian and must be updated annually. 

Required for any Scouting America event lasting longer than 72 hours, such as summer camp or extended backpacking and high-adventure trips. Part C requires a physical exam and a healthcare provider's signature. Please plan ahead, as scheduling a physical can take time. 

Used for specific troop outings that require explicit parental permission, such as shooting sports, aerial adventure parks, or specialized travel. We will notify the troop when this form is required for an upcoming event.
